STATE SCHOOL HEALTH REQUIREMENTS - Elementary

Illinois law requires that all students entering an early childhood program, kindergarten, sixth, or ninth grades, or transferring from out of state have a physical examination within one calendar year prior to the date of entry. The examination must include a complete immunization record and medical history. Students transferring from another Illinois school must present copies of their current Illinois health records. A Certificate of Child Health Examination, signed by a physician, must be on file before the first day of school for the school year. 
 
     Section 27-8.1 of the Illinois School Code states: "If a child does not comply…then 
     the local school authority shall exclude that child from school until such time as the 
     child presents proof of having had either the health examination or those required 
     immunizations which are medically possible to receive immediately." 

    Public Act 93-946 requires "all Illinois children in kindergarten, second and sixth 
    grades to have an oral health exam. A Proof of School Dental Examination Form, 
    signed by a licensed dentist, must be on file before May 15th for the school year."
 
    Illinois state law requires that "all children enrolling in kindergarten in a public, 
    private or parochial school and any student enrolling for the first time in a public, 
    private or parochial school shall have an eye examination."

Health Requirements - Elementary

Pre-K and Early Childhood Education Programs
  • Certificate of Child Health Examination including current immunizations, health history (completed by a parent/guardian) and physical examination that has been completed within the preceding 12 months from the first day of school.
Kindergarten
  • Certificate of Child Health Examination including current immunizations, health history (completed by a parent/guardian) and physical examination that has been completed within the preceding 12 months from the first day of school.
  • Eye Exam Report completed by an optometrist or physician who provides complete eye examinations within one year prior to the first day of school.  This may be substituted by an Eye Exam Waiver form in certain circumstances.
  • Proof of Dental Examination form completed within the preceding 18 months of the first day of school.  A Dental Exam Waiver form may be substituted in certain circumstances.
2nd Grade
  • Proof of Dental Examination form completed within the preceding 18 months of the first day of school.  A Dental Exam Waiver form may be substituted in certain circumstances.
6th Grade
  • Certificate of Child Health Examination including current immunizations, health history (completed by a parent/guardian) and physical examination that has been completed within the preceding 12 months from the first day of school.
  • Proof of Dental Examination form completed within the preceding 18 months of the first day of school.  A Dental Exam Waiver form may be substituted in certain circumstances.


The above health related items are required by Illinois for attendance in school. Forms are available at most health care provider offices, can be printed from the district website, or requested from the school nurse by email (branchfieldm@husd4.org) or by phone (309) 473-2822, ext 2002.
